This wrench is specially designed to install or remove screw-in-style water heater elements.
The smaller end will fit a standard 1-1/2 inch hex head used on most water heaters.
The larger end will fit the less common 1-7/8 hex heads used on some manufacturers’ heaters.
The side holes allow a screwdriver, or similar object, to be inserted and used for leverage.
